Assists individuals with serious and complex health care challenges to navigate large health care systems and find the resources they need to meet their health goals.
Fees: Medicaid pays for Health Homes Care Coordination if you are eligible. Hospital Care Coordinators work with whomever is referred to them by the hospital social workers.
